This section discusses how to determine the Program(s) for which a client is eligible.
The Clarity Program Eligibility Determination Engine streamlines Centralized Intake and Coordinated Assessment, making it easy and efficient. To determine if a client is eligible for a particular program, or to determine the program(s) for which a client is eligible, go to the Assessments tab in their client record.
Determining Program Eligibility
Each agency will have list of assessments targeted to measure the unique needs of their client base. Consult with your agency manager to determine which assessment is the most appropriate for your client, as the Eligibility Engine must be turned on for the assessment to determine eligibility.
To begin, Go to the Assessments tab and select the appropriate Start button to access the assessment.
Note: When completing a new assessment, note that some client data may be auto-filled using information from the client’s Profile Screen. Any changes you make to this information listed on the assessment main screen will not be applied to the Profile Screen – the purpose of the assessment is to capture a current snapshot of the client’s current situation.
Review Programs Listed
After clicking Save on the assessment, a list of programs will appear. This list will likely be divided into several categories (e.g. Agency Programs, Community Programs, etc.). In the example below, we see the VI-SPDAT Score Summary section as well. To view program details, select the down arrow.
Note that you also have the ability to print directions to the program site, as well as print a checklist of the required documents the client needs to bring with them to the program site.
This screen will also include programs for which your client is ineligible. To view these programs, click the down arrow to expand the Ineligible Programs section.
To determine the reason for ineligibility, select the down arrow. The box will drop down, providing several pieces of information, including reasons for ineligibility.
Refer to Community Queue
By clicking the Refer directly to Community Queue button, a referral is created and placed in the Community Queue. This referral is then visible to all Agencies within your CoC and available for fulfillment.
