This section discusses how to determine the Program(s) for which a client is eligible.
The Clarity Program Eligibility Determination Engine streamlines Centralized Intake and Coordinated Assessment, making it easy and efficient. To determine if a client is eligible for a particular program, or to determine the program(s) for which a client is eligible, go to the Assessments tab in their client record.
The program eligibility determination process is composed of three simple steps.
1. Select the appropriate assessment.
Each agency will have list of assessments targeted to measure the unique needs of their client base. Consult with your agency manager to determine which assessment is the most appropriate for your client. For the purposes of this example, the ‘Central Intake’ assessment will be used.
Select the ‘Start’ icon to begin the assessment.
2. Complete Each Assessment Field
Selecting ‘Start’ will bring you to the Assessment’s main screen. Here you will find that many of the fields have been conveniently auto-filled using information from the client’s Profile Screen. However, any changes you make to this information listed on the assessment main screen will NOT be applied to the Profile Screen – the purpose of the assessment is to capture a current ‘snapshot’ of the client’s needs.
3. Review Programs Listed
a. Eligible Programs
After selecting ‘Save’, a list of programs will appear. This list will likely be divided into several categories (e.g. Agency Programs, Community Programs, etc.).
To view program details, select the down arrow.
The following program information is displayed:
- Program Description
- Service Categories
- Target Population
- Housing Availability (with dropdown chart)
You also have the capability to print directions to the program site, as well as print a checklist of the required documents the client needs to bring with them to the program site.
b. Ineligible Programs
Note that the list will also include programs for which your client is ineligible.
To determine the reason for ineligibility, select the down arrow. The box will drop down, providing several pieces of information, including reasons for ineligibility.
