Program Chart Fields allow for longitudinal tracking of Program data. In the client record, the Chart feature can be accessed by opening a Program and selecting the Chart tab.
Note: It is possible to automate the creation of Chart Fields for repetitious programs. Some programs require the exact same Chart Fields, and the creation of these Chart Fields must be replicated when creating the programs, which can make program creation tedious and time-consuming. The Automated Provisioning feature saves time by eliminating the need to manually configure Goals (as well as Services/Service Items, Documents, and Chart Fields) when setting up such programs. Note that you must first create the Chart Field (process described below) before applying Automated Provisioning.
Managing Program Chart Fields
To manage Program Chart Fields, click on the Launchpad icon and select, Manage. Click on the Programs tab and mouseover the Program Name. Select the Edit link that appears to the left.
This will take you to the Modify Program screen. First check the toggle next to Program Charts to make sure it is enabled.
Click Save Changes and hen the Chart Fields section will be visible. Scroll down to the Chart Fields section and select the Add Field link.
A popup screen will appear listing the data fields that are included in the Enrollment screen. These are your options for Chart Fields. For more information on adding fields to Assessments, review the Adding Data Fields section of the Creating New Screens article.
Select Add and then click Save Changes at the bottom of the screen.
The selected field will now be documented with each status assessment, and it will be accessible through the Chart tab in the Programs screen, as shown below.
Each data point represents a point in time where the client's status has been measured. These status labels are described below.
|EN||This measurement point indicates the data at Program Entry|
|S(Number)||These measurement points indicate a Program Status point of measurement. For example S1 represents the first status assessment, S2 would represent the second, and so on.|
|AS||This measurement point indicates that the data was collected during an annual assessment.|
|EX||This measurement point shows the data point measurement at the time of Program Exit|
|F(Number)||These measurement points indicate a Follow-up point of measurement. For example F1 represents the first Follow-up assessment, F2 would represent the second, and so on.|
