Report Purpose & Summary
This program-based report provides program outcome information that includes program client exit information, housing status of exited clients, and efficiency/process measures. It also provides agency participation totals, and unduplicated counts of clients exited from either each program category and/or collectively.
The following parameters are available to obtain targeted details in the [OUTS-101] Program Outcome Measures report:
|Select a Category||Select one or more program categories to include in the report. Select ‘Summary’ to generate information on all of the program categories collectively; information will include program outcome percentages, total clients served by program category, and agency participation totals.|
|Select a Program||Select the program(s) that coincide with the selection in the ‘Select a Category’ dropdown menu.|
|Choose Start Date||This date will reflect the date from which you would like to start generating data.|
|Choose End Date||This date will reflect the date you would like to stop generating data|
|Choose Report Format||You can select Web Page, PDF, or Excel. A Web Page is a functional format that allows you to drill down on data elements to inspect the underlying data supporting it. The PDF is a good option for official reports, grants, etc. If you download the excel file you can can use excel to manipulate the data (e.g. Sort/Check for duplicates, etc.)|
This program-based report provides the following pieces of information:
- Program outcome percentages for a variety of client exit information as well as efficiency/process measures
- Unduplicated counts of clients served for each program type
- Agency reporting participation for each program type
- Exit information for clients served including income, destination, and housing data
This report has two formats:
- Summary Format: Provides the following for all program categories: Program Outcome Percentages, Total Served By Program Type, Agency Reporting Participation. To generate a Summary format, select ‘Summary’ in the first dropdown menu on the report parameter screen.
- By Program Category: Provides details on 6 program outcome measures. To generate information according to program category, select one or any combination of program types from the first dropdown menu on the report parameter screen.
1. Summary Format
1.1 Program Outcome Percentages Table
This table has Program Categories (Horizontal Axis) and Outcome and Efficiency/Process Measures (Vertical Axis).
1.1a. Total Exits
The row titled Total Exits indicates the number of clients exited from the corresponding program type during the report dates. This information is derived from the program exit screens of each program.
The items listed in the Outcomes column are derived from the data fields included in a standard program exit screen.
1.1c. Efficiency/ Process Measures
- Occupancy: These percentages represent percentage of occupancy for Emergency Shelters and are derived from client totals documented using Attendance. These totals are an average of utilization for the reporting period. This field is null for other program types.
- Exit to Known Destinations: This is a percentage of exited clients who reported a known destination upon exiting the program. This information is derived from the Destination field of a program exit screen.
1.2 Total Served By Program Type Table
This box provides totals for client exits by program type, and an unduplicated total of all clients exited from all program types. Note that this total will be less-than or equal-to the total sum of each count for program types. This is because it is an unduplicated total, and thus accounts for clients who were exited from 2 or more programs during the report dates.
For example, in the screenshot above, the Total = 18, but the Total Unduplicated Served = 12. This means that one or more clients were enrolled in more than one program.
1.3 Agency Reporting Participation Table
This indicates which agency is reporting their participation in HMIS data collection for each program type.
2. By Program Category Format
This format shows 6 program outcomes for selected program(s) within a single program category, as opposed to providing a general summary of all program categories, as with the Summary Format described above.
6 Outcome Measures:
- Obtain permanent housing
- Decrease exits to streets/shelter
- Exit with earned income
- For those who had 0 income at Intake, obtain stable income before discharge
- Exits to Known Destinations
- Of those who discharged to permanent housing, length of time to obtain housing?
I. Obtain Permanent Housing
This outcome measure shows how many clients obtained permanent housing before exiting the program.The shaded rows are considered ‘Permanent Housing’
This information is derived from responses to the Housing Status at Exit and Destination data fields in the program exit screen. Note that each row corresponds with a response option on the Destination dropdown menu (screenshot below):
II. Decrease Exits to Street or Shelter
This table shows what percentage of clients exited to the street or to a shelter. The percentage is the total amount of street/shelter exits out of the total amount of program exits (not total amount of clients exited from a program). So, in the screenshot below, out of 10 program exits, 4 clients reported a destination of the street or a shelter.
This information is also derived from responses to the Housing Status at Exit and Destination data fields in the program exit screen. Clients who report Emergency Shelter or Place Not Meant For Habitation in the Destination dropdown menu are included in the total.
III. Exit with Earned Income
This outcome measure tallies how many clients exited the program with earned income. It also reports employment status for exited clients.
Income Source At Exit: This tallies that amount of clients who reported income source(s) at exit. This information comes from the Earned Income data field in the program exit screen. All clients who report ‘Yes’ to this question are included in this client count.
Of those discharged, how many were employed at exit?: This provides percentages (out of total program exits) for all employment statuses. This information is derived from the Employed data field on the program exit screen.
IV For those who had 0 income at Intake, obtain stable income before discharge
This table reports an unduplicated count of those clients who obtained a stable income by their program discharge, and also tallies the different sources of income at program discharge.
This information is derived from the program exit screen. If a client reports any of the Cash Income Sources listed in the program exit screen, they are included in the ‘Total Unduplicated Adults that obtained stable income before discharge’ count. This total is then compared to the total amount of clients who reported no cash income at program entry.
The system then tallies which income sources were reported by the clients discharged. Notice that the Total may be equal to or greater then the Total Unique Count (9 and 3, respectively, in the screenshot above). This is because one client may report more than one source of income.
V Exits to Known Destinations
This table reports the percentage of clients who reported a known destination at program exit.
This table counts the amount of clients who chose the ‘Don’t Know’ response option of the Destination data field in the program exit screen.
VI Of those who discharged to permanent housing, length of time to obtain housing?
This table reports the length of time to obtain housing. This information is derived from program goals and/or by the date of program exit for all clients who obtained permanent housing.
Drilling down on the totals for Total Served fields to generate the client list that comprises each total.
